Because there are many aspects of a sleep problem it takes a tremendous amount of time to investigate all the details. Most health-care professionals simply don't have the time to ask about all the aspects. This is why the Sleep/Insomnia Questionnaire was developed.
There isn't enough time in an ordinary appointment with a health professional to cover all the aspects included in this questionnaire.
Talking about a sleep problem is a sensitive area because you are vulnerable when asleep and sleep habits are so private. This questionnaire was developed by a mental health counselor with expertise in talking to people about tender issues. It will add comprehensiveness with compassion to the investigation of your sleep problem.
Several hundred people have spent hours of their time filling out the Forms Version of the Sleep Questionnaire. There are 202 questions that are used to create the following report.
Prepared by a counselor trained in sleep/insomnia problems. This report is a throrough review of your answers, opinions of indications, suggestions of options, and questions that appear to be important to getting closer to a good solution to the problem. This report costs $60.
The questionnaire is available as text. You can save it to your own disk and then fill out on your own time. You can then share your responses with your health care-provider. While they aren't organized into categories, your answers can still help to point out the way to a customized method of treatment.
